SOHS Band Rehearsal Absence Form
This form should be submitted for any rehearsal you will be absent from, be late for, or need to leave early from. It does not need to be filled out if you are absent from school for illness.
THIS FORM SHOULD BE SUBMITTED AS EARLY AS POSSIBLE BUT NO LATER THAN 48 HOURS PRIOR TO ABSENCE UNLESS IT IS AND EMERGENCY CIRCUMSTANCE
PLEASE REALIZE THAT MISSING ANY REHEARSAL IS DETRIMENTAL TO THE SUCCESS OF OUR BAND AND MAKE EVERY EFFORT TO AVOID MISSING REHEARSAL
